Bokaal van kleurloos glas. De kelk heeft een platte voet en een peervormige stam met een grote luchtbel. Hierboven twee ringen. Op de recht uitlopende kelkwand was een afbeelding gedrild van een zeemeermin die viool speelt. In de bovenrand de tekst: "Formosa Superbe". De bokaal is een veel stukken gebroken en deels weer in elkaar geplakt.
